Ermineskin Cree Nation (ECN) Chief and Council are interested in understanding the needs, preferences, and perspectives of Nation members relative to the design and delivery of continuing care. Chief and Council are aware of a need to explore options for our members, and as we continue engaging in discussions around this important topic, firsthand community member input is invaluable.
*For this survey, continuing care includes the services below:
-Home Care (Assisted Living)
-Designated Supportive Living
-Long-Term Care
-Hospice

• Assessment Tool: The survey is a preliminary step designed to evaluate various aspects such as demand, location preferences, and potential challenges. It is crucial for guiding initial discussions and planning.
• No Guarantee of Development: Participation in this survey does not guarantee that a facility will be approved or built. The survey results will be one of several factors considered in the decision-making process.
• Anonymous Responses: All responses will be kept anonymous. We do not collect any personal information that can identify you, and your answers will remain confidential.
• Optional Prize Entry: If you would like to enter our prize draw, you have the option to provide your name and contact information at the end of the survey. This is entirely optional, and your information will be kept separate from your survey responses and used solely for the prize draw.
